چکیده

In patients with asthma, is a management strategy that controls lower airway eosinophilic inflammation and symptoms more effective than standard care for reducing asthma exacerbations? Randomized {allocation concealed*} †, blinded (clinicians and patients),* controlled trial with 12-month follow-up. P a t i e n t s 74 patients (54% men, age range 18 to 75 y) who had moderate to severe asthma and probably needed continued hospital follow-up. Exclusion criteria included current smokers , a smoking history of > 15 pack-years, clinically important comorbidity, poor compliance with treatment, aggravating factors that were inadequately controlled (e.g., rhinitis), and a severe exacerbation within 4 weeks of entry to the trial. Follow-up was 92%. 37 patients each were allocated to management with reference to the induced sputum eosinophil count (eosinophil group) or management by a modified version of the British Thoracic Society guidelines (BTS group). In the eosinophil group, decisions about anti-inflammatory treatment were made in accordance with an algorithm based on maintenance of a sputum eosinophil count < 3% with a minimum dose of antiinflammatory treatment. In the BTS group, treatment decisions were based on usual assessments of symptoms, peak expiratory flow, and use of β 2-agonists. Number of severe asthma exacerbations (defined as a decrease in the morning peak expiratory flow to > 30% below the baseline value on ≥ 2 consecutive d or deterioration in symptoms needing treatment with oral cor-ticosteroids) and control of eosinophil airway inflammation measured by the induced sputum eosinophil count. Analysis was by intention to treat. Over 12 months, the number of severe exacerbations was lower in the eosinophil group than in the BTS group (35 vs 109 total exacerba-tions, P = 0.01). The sputum eosinophil count was 63% (95% CI 24 to 100, P = 0.002) lower in the eosinophil group than in the BTS group. Fewer patients in the eosinophil group than in the BTS group were admitted to hospital because of asthma exacerbations (1 vs 6, P = 0.047). The groups did not differ for average daily dose of inhaled or oral corticosteroids. In patients with asthma, a management strategy that controls lower airway eosinophilic inflammation and symptoms was more effective than standard care for reducing asthma exacerbations. Asthma exacerbations and sputum eosinophil counts: a randomised controlled trial. C o m m e n t a r y Most physicians agree that inflammation plays a pivotal role in the pathophysiology of asthma. Asthma exacerbations and sputum eosinophil counts: a randomised controlled trial.

BACKGROUND Treatment decisions in asthma are based on assessments of symptoms and simple measures of lung function, which do not relate closely to underlying eosinophilic airway inflammation. We aimed to assess whether a management strategy that minimises eosinophilic inflammation reduces asthma exacerbations compared with a standard management strategy. METHODS We recruited 74 patients with ...

Cough, airway inflammation, and mild asthma exacerbation.

BACKGROUND Prospective data on the temporal relation between cough, asthma symptoms, and airway inflammation in childhood asthma is unavailable. AIMS AND METHODS Using several clinical (diary, quality of life), lung function (FEV(1), FEV(1) variability, airway hyperresponsiveness), cough (diary, cough receptor sensitivity (CRS)), and inflammatory markers (sputum interleukin 8, eosinophilic ca...

Eosinophilic airway inflammation and exacerbations of COPD: a randomised controlled trial.

Evidence suggests that eosinophilic airway inflammation is important in the pathogenesis of severe chronic obstructive pulmonary disease (COPD) exacerbations. The present authors tested the hypothesis that a management strategy that aims to reduce sputum eosinophil counts is associated with a reduction in exacerbations of COPD. A total of 82 patients with COPD were randomised into two groups. O...

Managing asthma in pregnancy

UNLABELLED Asthma is a common comorbidity during pregnancy and its prevalence is increasing in the community. Exacerbations are a major clinical problem during pregnancy with up to 45% of women needing to seek medical help, resulting in poor outcomes for mothers and their babies, including low birth weight and preterm delivery. The goals of effective asthma management in pregnancy are to mainta...

Subclinical phenotypes of asthma.

PURPOSE OF REVIEW Asthma is a heterogeneous disease. Identification of specific subphenotypes of asthma may further our understanding of pathophysiology and treatment response, leading to the better targeting of both existing and novel antiasthma therapies. An accurate and comprehensive clinicopathological classification system therefore remains an important priority for asthma research. The pr...
